Uncircumcised males...

My son is 5 1/2 mos and not circumcised. I had a family member watch him the other day, and I am concerned. When I picked him up and changed him I noticed his skin was extended like someone pulled it back. Then the next day I noticed what looked like a small tear on the inner part of his skin when bathing. Should I be concerned? I asked this person who is very trustworthy and they knew nothing about it. Is this normal... or should I call the dr? Thanks in advance...
