My son is 5 1/2months and is not circumsized. What you are describing is in my opinion not normal. I would definitly go see the doctor. On another note my grandmother (85 yrs old) keeps tellins me that her doctor pulled the skin back on my dad by giving it a yank as she puts it.. this was 65 yrs ago !! Not a practice that we do nowadays... yet she insists this is what I should be doing. So could it be that the person you left him with was trying to clean it in a way we are advised not to nowadays? Either way if it was me I would be looking into it further.
