a few questions

I have a little boy 20mo old. I'm wondering a few things...
1. when should I give him a pillow to sleep with?
2. when should I move him to a toddler bed?
3. how can I get him to brush his teeth completly? He won't open his mouth. He'll "brush" his teeth..mostly chew on the brush, but I don't think he's getting them clean.

any suggestions??
