Sleep problem

My 15 month old wakes up four or more times every night. He wakes up instantly and starts screaming and crying. Sometimes he's up for hours crying inconsolably. Other times he'll go to sleep quickly, but only if I nurse him. I want to know what is causing him to wake up. Is it nightmares? He has eczema, could it be itching? I've tried altering the temperature in the bedroom, kicked out the cats, used anti-itch cream, given him heavier meals before bed and lighter meals, etc. Nothing seems to be working. I've used Benadryl a few times and that helps a lot, but I don't want to have to drug my child to sleep every night! Has anyone dealt with this? What worked for you? Thanks.
