I'm no expert on 4-year-olds but it seems to me that you'll need to make sure he's not hurting himself, and just walk away. Not literally, mind you, but just don't give in to his tantrum. Eventually once he figures out he won't get his way by screaming and kicking, maybe he'll stop.
