Both of my girls (age 5 and 7) have an underbite. I have taken them to an orthodontist who is recommending treatment for the 7 year old because she has enough adult teeth. My understanding is that treatment for an underbite can be done around that age or at 18 when they have to break the jaw. I am going to get another opinion and am researching on-line. By the way, when my 7 year-olds permanent teeth came in, the underbite was much less pronounced.
