Help baby won't gain weight!!

I am the mother of a 4 month old. She has only gained 1 1/2 lbs in the four months. We have tried everything we can think of. She is constantly spitting up or vomiting. I am tired of putting her through tests which find nothing and giving her medicine that is not helping. We have also tried adding cereal to the formula. But nothing seems to help. any suggestions please email me or respond.
