I had the exact same problem with my 2 kids, and I know what caused it...I held them ALL THE TIME! LOL now that may not be your problem ,but it was mine! What can I say I loved holding them! BUt I finally broke down and bought a teddy bear that makes "womb" sounds from Walmart, I am not sure if other stores carry them but they might. It worked for me, maybe it will work for you! Good luck and keep us posted!

: The biggest problem is what you mentioned .. that she falls asleep in your arms. If you put her down asleep she'll wake up and wonder how she got where she is. Try rocking/walking/whatever with her until she's almost asleep, or if you have to wake her up a little before putting her down. This was a big problem we had with my little girl. Good luck!
