I don't know how old your baby is, but I remember when my daughter was about 2-6 months old she sometimes did what you're explaining. Maybe your baby is adjusting to new sleep times. Sometimes I would just leave her for up to 10-15 minutes to see if she could put herself back to sleep, sometimes she did, and sometimes she didn't. Sometimes I would rock her back to sleep if I thought that her nap was too short and if she was still sleepy. That helped, and then eventually she would sleep longer.
