Steve, I know exactly what you are going through. I've been going through somewhat the same thing with Matthew. He was always falling asleep on the breast and I would sneak him into bed.. so I started on the settling plan we've all been talking about here with mixed results. Matthew is EXACTLY the same as Hannah. He just cries and screams and chokes until one of us picks him up, all the patting and whispering doesn't seem to help.

Like you my husband and I have been letting him cry while every few minutes going in to reasure him that we are right there ( heartbreaking task i'm sure is harder on us than on him ) Every night its been less and less screaming however its heartwrenching to hear him cry and sob, as well as being torn between wondering if we are doing more harm than good.
I have to wonder though how bad the 'falling asleep on the breast' really is though as it was putting him at peace and allowing him to fall asleep peacefully and at ease. Who knows ? I'm perplexed and just hope that I'm not causing the little chap too much distress at bedtime. We'll see what happens over the next few days and if it doesn't get much better I may just go back to nursing him again.
Anyway, good luck for tonight and let me know how it goes.
