Yes, it is VERY unusual to sign a form like this. I have been to 3 RE's and not one has made me sign anything like this. I've even discussed clomid with them in great detail and they ALL said that it is NOT linked to ovarian cancer. The "possible" link as your document says was done in a study in the 70's. Doctors saw a higher link with ovarian cancer and women who took clomid. That study had been proven false as they had found out that it was, NOT OVULATING, which caused ovarian cancer - not clomid. Because most women who are on clomid have not ovulated in some time, that is where the link has come in. Clomid is safe.
The reason your doctor doesn't keep you on it for more than 3 months is NOT because it is dangerous it is because if after 3 months it does not work for you it will NEVEr work for you. Why stay on a medication that doesn't work? The only time that clomid can be bad for you is if you are on it for a long period of time. BUT that is the case with any and all medication. You can't be on prednisone (sp?), or antibiotics for an extended amount of time either w/o horrible effects. People still take it though, just in moderation.
Also, it is very rare to hyperstimulate on clomid. In fact, I have never heard of a clomid case where this is true. As your doctor if s/he has. That is more for injectibles.
I know that I must sound crazy to keep posting about this, but it is frustrating to see posts where someone has the wrong facts.
